He thus proposes the use of an "analytical matrix" aimed at allowing the actors involved in conflictive situations (always in a self-analytical position) to achieve a broader understanding of such conflicts. There would be new possibilities for contractibility in the management of the health service's daily routine, as well as new shapes in the relations among workers; this would include bringing previously concealed conflicts to the surface and helping them reach the service's decision-making arena. | |  | Starfield, Barbara | Atención primaria y responsabilidades de salud pública en seis países de Europa y América del Norte: un estudio piloto read moreAbstract: Background: Rapidly occurring changes within the health care systems are creating an opportunity to re-orient the relationships between their different sectors. In order to know the locus of responsibility for various types of preventive activities, we undertook an inquiry on eight areas in six countries from Europe and North America. Methods: An inquiry among experts based on a matrix which arrayed the type of preventive health services against the target population. Eight clinical conditions were identified (childhood immunizations; adult influenza vaccination; mammography screening, tuberculosis screening, hypertension screening, PKU screening, HIV screening, and osteoporosis testing) trying to know their target population and the locus of responsibility for setting of policy, level to contact individuals for testing, follow-up of people with abnormal tests and maintenance of their medical records. Results: This pilot study showed very little results coincidence either within the eight surveyed areas or across them. There was no regular pattern for the preventive activities studied among the different countries, neither according to the type of health system, nor to the primary health care orientation of the different systems. Conclusions: There was a limited consensus in the activities studied concerning the best mode of doing public health interventions for personal health services. | 2004 |
| |  | Trevizan, Maria A. | Al encuentro de la competencia del cuidado según Boff: una nueva perspectiva de conducta ética de la enfermera gerente read moreAbstract: En el presente ensayo, los autores caracterizan la conducta gerencial de la enfermera en la perspectiva del modo-de-ser-trabajo, ilustrada a través del análisis de tres investigaciones. Entendiendo la fragilidad de la situación gerencial de la enfermera en términos de compromiso con los usuarios y prestadores de servicios, los autores toman posición a favor de la una intervención que corrija ésta actuación. Es con éste objetivo que proponen una alternativa a través de la cual se puede reflexionar sobre la corrección necesaria. Se trata de una nueva ética que articula un nuevo sentido de actuar fundamentado en el modo-de-ser-cuidado propuesto por Boff, en donde la relación es de convivencia, interacción y comunión. Según éste modo-de-ser, el cuidar de la enfermera gerente se torna una actitud de preocupación, responsabilidad y compromiso afectivo con el otro. En este sentido, la razón instrumental abre espacio para la razón sensible y cordial, el espíritu de delicadeza y el sentimiento profundo. A partir de estos nuevos valores la enfermera pasa a comprender mejor la dimensión de la alteridad, de la acogida, del respeto, de la reciprocidad y de la complementariedad en la relación gerencial. | 2003 |

To construct useful indices for hospital management, based on descriptive multivariate techniques. Material and Methods. Data were collected during 1999 and 2000, on hospital admissions occurring during 1997-1998 at Hospital General de Algeciras, part of Servicio Andaluz de Salud (SAS) of Sistema Nacional de Salud Español (Spanish National Health Service). The following variables routinely monitored by health authorities were analyzed: number of admissions, mortality, number of re-admissions, number of outpatient consultations, case-mix index, number of stays, and functional index. Variables were measured in a total of 22486 admissions. We applied the Principal Components Analysis (PCA) method using the R correlation matrix. Results. The first two components were selected, accounting cumulatively for 62.67% of the variability in the data. Conclusions. The first component represents a new index representing the number of attended persons, which we have termed Case Load. The second PC represents or explains the difficulty of the attended cases, which we have termed Case Complexity. These two indices are useful to classify hospital services. | 2002 |

Examina-se, em particular, a hipótese de modelos de gestão democráticos e participativos, com forte publicização da vida institucional, estarem sendo vistos como controlistas pelos trabalhadores, o que seria um aparente paradoxo com seu discurso de criação de autonomia. A criação de múltiplos dispositivos ou contextos de negociação, nos quais se encontram e se confrontam as múltiplas racionalidades organizacionais, é apontada como alternativa para uma gestão radicalmente democrática, apoiada na idéia de que nenhum macroator (a direção ou o governo) tem o poder de imprimir a racionalidade da organização, sem enfrentar um complexo, permanente e dialógico processo de negociação que envolveria todos os atores organizacionais. | 1999 |
